Providence Health & Services RN Clinical Liaison in Centralia, Washington
Providence is calling a RN Clinical Liaison (1.0 FTE, Days) for Home and Community Care (PHCC) in Centralia, Washington.
The Clinical Liaison position is responsible for creating and maintaining positive customer relationships in order to increase referrals for Home Health, Hospice, Transitional Care Unit and other services within assigned accounts. This role includes working with discharge planners, facility staff, physicians, and individuals to assess appropriateness for hospice and home health services, and facilitate the coordination of services and to ensure there is appropriate documentation for compliance with agency and statutory guidelines to manage the transition of the patient from an acute care facility to the patient's home or next location of residence. Consultative functions include, but are not limited to: providing consultation and informational presentations regarding end of life care and home health; sharing expertise through in-services on pain and symptom management options; serving as a clinical resource and educator for physicians, facility staff, patients, families and community on Home Health or other issues in various settings and facilities.
In this position you will:
Maintain and build existing referral base relationships by serving as a resource for education and information.
Evaluate referrals received on hospitalized patient from a variety of care settings for appropriateness for Home Health and/or other in-home services programs as appropriate.
Provide information to patients and their families interested in Home Health.
Participates, when appropriate, in health care facility care conferences regarding specific patient consult requests.
Coordinate discharge planning for Hospice and Home Health patients who have been admitted to the hospital and act as a liaison to Ministry staff for smooth transition.
Accept referrals from hospital discharge planner after they have identified patient agency choice and type of home service needed.
Coordinate with the Inpatient Hospital Team and primary RN/MSW to ensure discharge planning is comprehensive and communicated efficiently.
Collaborate in Care Plan development, implementation and evaluation with discharge planners. Participates, when appropriate, in discharge planning rounds in health care facilities.
Appropriately document activities in the Providence electronic medical system; tracks referrals received by nursing unit and accepted by each Providence agency.
Maintains open positive communication with appropriate agency staff regarding referrals received.
Effectively manages customer resolution issues independently or seeks management/leadership involvement as needed.
Coordinate with other team members to support referral generation efforts; facilitate growth of Home Health and promote Providence Home and Community Care Ministries according to business development strategies.
Actively participate in the development and execution of strategic initiatives that include referral generation and business growth in assigned accounts.
Respond to daily needs of local Team, including facilitation of excellent communication channels between Business Development and Intake Department.
Serve as a local champion for service-line standardization and consistency efforts.
Enter orders and document in Epic EMR.
Required qualifications for this position include:
Associate's degree in Nursing
3 years of experience in clinical discipline.
Current Washington State RN licensure.
Current State Driver's license.
Current Healthcare Provider CPR certification.
Preferred qualifications for this position include:
Bachelor's Degree in Nursing or other clinical areas related to service line offerings
Liaison, marketing or sales experience.
Health care industry experience.
Documented prior successful sales account medical management experience.
Epic EMR experience.
